Pacific Health Group in California is seeking a Licensed Vocational Nurse (LVN) for the Enhanced Care Management (ECM) team in Hiring County. You will support high‑risk Medi‑Cal members with complex medical, behavioral health, and social needs under RN supervision.

The role combines remote work with field visits, care coordination, education, and documentation to improve health outcomes and reduce barriers to care for vulnerable populations.

Qualifications

• Current California LVN license and active practice.
• Minimum 1 year of clinical or care coordination experience preferred.
• Experience serving underserved populations is a plus.

Responsabilités

• Support care coordination for ECM program.
• Assist with member assessments, education, and medication reconciliation.
• Coordinate transitions of care and referrals with providers.
• Conduct field visits and remote support to high‑risk members.
