Job summary

BLEHEALTH, LLC seeks a Lead Care Manager (LVN) licensed in California to collaborate with chronically ill members, families, and providers in a team-based care model. You will coordinate services, engage eligible members, oversee ECM implementation, and travel as needed.

You will promote access to care, monitor treatment adherence, provide self-management training, coordinate with hospital discharge teams, and connect members to community resources, with emphasis on culturally and linguistically

Qualifications

• Current LVN licensure in the State of California
• Proficiency in communication technologies (email, cell phone, etc.)
• Highly organized with the ability to keep accurate notes and records
• Experience with Health IT systems and reports is desirable
• Local knowledge about and connections to community health care and social welfare resources are desirable

Responsibilities

• Coordinate with members, families, and providers to ensure seamless services
• Engage eligible members and oversee ECM services and care plan implementation
• Offer services where the member lives or seeks care within Medi-Cal MCP guidelines
• Monitor treatment adherence, provide health promotion and self-management training
• Promote timely access to appropriate care and reduce ER visits and hospital readmissions
• Educate members and families about community resources and coordinate transitions in care
