Blehealth,-LLC is seeking a Lead Care Manager (LVN) to coordinate care for chronically ill or high-risk members in a remote role with periodic on-site requirements. You will collaborate with providers, family caregivers, and community resources to implement the care plan and ensure seamless service delivery.

The role emphasizes engagement, adherence monitoring, health promotion, and reducing unnecessary ER visits, with an emphasis on trauma-informed and culturally sensitive care.

Qualifications

• Must hold current LVN license in California.
• Proficiency with email and mobile tech, and good record-keeping.

Responsabilités

• Coordinate and collaborate with care teams to implement care plans.
• Engage eligible members and coordinate with providers and community resources.
• Monitor adherence to care plans and facilitate timely access to care.
• Promote health promotion and self-management training.
• Assist with transport arrangements and discharge planning as needed.
