Job summary

BLEHEALTH, LLC seeks a Lead Care Manager (LVN) to collaborate with chronically ill members and their families, providers, and community resources in a team approach. You will coordinate services, engage eligible members, oversee ECM implementation, and advocate for patients within Medi-Cal MCP guidelines.

The role requires a California LVN license, strong organizational skills, and bilingual abilities; occasional in-person reporting may be required at a satellite location.

Qualifications

• Current LVN license in California is required.
• Proficiency with email and mobile devices.
• Highly organized with accurate record-keeping.
• Bilingual abilities are a plus.
• Experience with health IT systems desirable.

Responsibilities

• Coordinate with members, families, providers, and staff to ensure seamless care.
• Engage eligible members and oversee ECM services per care plan.
• Accompany members to visits and monitor treatment adherence.
• Connect members to social resources, transportation, and community supports.
• Maintain timely communication with care team and payers.
