The position is primarily remote with periodic satellite office/onsite duties, requires a valid driver's license and a personal vehicle, and includes mileage reimbursement. 30–32 USD per hour begins the compensation range.

Qualifications

Responsibilities

• Coordinate with member, family/caregiver, and providers to ensure seamless care and avoid service duplication.
• Engage eligible members and oversee ECM services and care plan implementation.
• Offer services in member's home or accessible locations per plan guidelines.
• Connect members to social services and transportation when needed.
