Job summary

Blehealth, LLC seeks an LVN-licensed Lead Care Manager to collaborate with care teams, families, and providers across settings. You will oversee ECM services, develop and monitor care plans, and engage high-risk members at home or in facilities in line with Medi-Cal MCP guidelines.

The role emphasizes advocacy, education, and coordination with social supports, including transportation, to improve outcomes and reduce hospital utilization.

Qualifications

• Current LVN license in California required.
• Experience with health IT systems is desirable.
• Local knowledge of community health care and social services is a plus.
• Proficient with email and mobile communications.

Responsibilities

• Coordinate with members, families, and providers to ensure seamless care.
• Engage eligible high‑risk members and oversee care plans.
• Advocate for members with healthcare professionals and payers.
• Accompany members to appointments and monitor adherence to plans.
• Connect members with social services, transportation, and resources.
• Utilize motivational interviewing and trauma‑informed approaches.
• Collaborate with hospital staff on discharge plans and transitions.
