Job summary

Pacific Health Group is seeking a Licensed Vocational Nurse (LVN) to join the Enhanced Care Management (ECM) team in Hiring County. The LVN will support high-risk Medi-Cal members with medical, behavioral health, and social needs, under RN supervision.

Combining remote work with field visits, the role involves care coordination, patient education, documentation, and collaboration with nurses, CHWs, providers, and community partners to improve outcomes and reduce barriers to care.

Qualifications

• Excellent communication, organizational, and interpersonal skills are required.
• Ability to work independently and in interdisciplinary teams is essential.
• California LVN license and relevant clinical experience are required for this role.

Responsibilities

• Coordinate care and assist with member assessments and education.
• Support medication reconciliation and transitions of care under RN supervision.
• Conduct field visits and collaborate with healthcare providers and community partners.
