Job summary

Pacific Health Group in California is seeking a Licensed Vocational Nurse (LVN) to join the Enhanced Care Management (ECM) team in Hiring County. You will support high-risk Medi-Cal members with complex medical, behavioral health, and social needs, conducting assessments, care coordination, education, and medication reconciliation under RN supervision.

Qualifications

• Valid California LVN license and unrestricted practice.
• Minimum 1 year of clinical experience in healthcare, managed care, home health, public health, or related settings.
• Experience serving underserved populations preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct health screenings and assessments within LVN scope.
• Collect and document health information, vital signs, medications, and observations.
• Assist with chronic disease management and education.
• Perform medication reconciliation under RN supervision.
• Coordinate field visits and community-based care coordination.
• Document accurately in the electronic health record and care management systems.
