Job summary

Tuesday Health is seeking a Licensed Vocational Nurse in Dallas, TX to join our value-based palliative care team. You will work with nurse practitioners and RNs to administer assessments and help develop coordinated care plans in a patient-centered model.

This role emphasizes empathy, strong communication, and the ability to use electronic systems to support the member journey, with remote work options and periodic onsite sessions.

Qualifications

• Licensed Practical Nurse with Texas license in good standing.
• Strong communication and organizational skills.
• Experience in clinical/medical settings preferred.
• Ability to learn and use multiple documentation systems.

Responsibilities

• Administer clinical assessments and screenings with the care team.
• Prioritize needs based on assessments and coordinate with the interdisciplinary team.
• Lead care plan development within the multidisciplinary team.
• Participate in rounds to ensure a coordinated member experience.
• Learn and use Tuesday Health electronic systems for coordinated care.
• Build trusting, empathetic relationships with members.
• Act as quarterback of the multidisciplinary team for complex care.
