Job summary
Tuesday Health in Dallas, TX seeks a Licensed Vocational Nurse to join our value-based, palliative care team. The LPN collaborates with nurse practitioners and RNs to deliver coordinated assessments, develop care plans, and support patients and families along the illness trajectory.
The ideal candidate holds an LPN license in Texas, has clinical experience in multidisciplinary settings, strong communication and organizational skills, and comfort with learning new electronic systems.
Qualifications
- Active and unrestricted LPN license in Texas.
- Clinical/medical setting experience preferred.
- Experience in a multidisciplinary care model.
- Strong communication and organizational skills.
- Ability to learn and use electronic systems.
Responsibilities
- Administer assessments and screenings with NP/RN support.
- Prioritize needs and contribute to care plans.
- Develop and implement individualized care plans.
- Participate in care rounds to optimize member experience.
- Collaborate with the interdisciplinary team to coordinate care.
- Build trusting relationships with members through empathy and listening.
