Job summary

Tuesday Health is seeking a Licensed Vocational Nurse to join our Complex Care Navigator team in Dallas, TX. You will work with nurse practitioners and RNs to conduct assessments, develop and implement individualized care plans, and participate in care rounds to optimize the member experience.

Strong communication and empathy are essential as you coordinate care across disciplines using our electronic systems.

Qualifications

• Active and unrestricted LPN license in the State of Texas.
• Experience in clinical/medical settings preferred.
• Strong communication and organizational skills; ability to multitask.

Responsibilities

• Administer clinical assessments and screenings within the care model.
• Prioritize needs and contribute to care plan development with the multidisciplinary team.
• Participate in care rounds to optimize member experience and coordinate care.
• Learn and use Tuesday Health electronic systems and tools for coordinated care.
• Build trusting relationships with members through listening and empathy.
