Tuesday Health is seeking a Complex Care Navigator RN to lead person-centered care for individuals with serious illness. You will administer assessments, guide care planning, and coordinate multidisciplinary rounds to optimize the member experience.

Ideal candidates are Texas-licensed RNs, bilingual in Spanish, with strong communication skills and proficiency in EHR systems. Remote work is available with periodic on-site sessions in Texas.

Qualificações

• Active Texas RN license with no board actions.
• Bilingual in Spanish.
• Experience in clinical/medical settings preferred.
• Experience in multidisciplinary care settings and cross-network collaboration.
• Strong communication and organizational skills.

Responsabilidades

• Administer and interpret assessments and screening tools across the care timeline.
• Develop individualized care plans within the multidisciplinary team.
• Lead internal and joint rounds to optimize member experience.
• Learn and utilize Tuesday Health electronic systems and tools.
• Build trusting relationships with members to support coordinated care.
• Serve as quarterback of the team to transform care for seriously ill members.
