Job summary

Molina Healthcare is seeking an RN care manager to support LTSS and care coordination across Dane, Dodge, Jefferson, and Rock Counties. This remote role requires travel to member sites, comprehensive assessments, and collaboration with a multidisciplinary team to ensure outcomes and cost-effective care.

Required are RN licensure, 2+ years in health care with LTSS or related fields, and ability to work across diverse settings.

Qualifications

• RN license active and unrestricted in state of practice.
• Experience in care management, managed care, LTSS, or equivalent.
• Experience with individuals with disabilities, chronic conditions, and substance use disorders.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and care team.
• Monitors care plans and documents interventions and outcomes.
• Promotes integration of behavioral health and LTSS services.
• Assesses medical necessity and authorizes waiver services.
