Qualifications

• At least 2 years in health care; LTSS experience preferred.
• Licensure/certification required only if state contract mandates; active in state of practice.
• Some states require a health-care related bachelor's degree.
• Valid driver’s license, reliable transportation, and auto insurance for travel.
• Experience with substance use disorders preferred in some states.
• Proficiency with Microsoft Office and navigating online portals/databases.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and care team.
• Monitor care plans and adjust interventions to meet goals.
• Promote integration of LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize waiver services.
• Coordinate interdisciplinary care team meetings and document outcomes.
• Travel 25–40% locally as required.
