Job summary

Molina Healthcare is seeking a care manager RN to support LTSS-focused care coordination across Dane County, WI. You will collaborate with multidisciplinary teams to ensure high-need members achieve outcomes through comprehensive assessments, care plans, and coordinated services.

Qualifications

• RN license active and unrestricted.
• 2+ years in health care with care management or LTSS exposure.
• Experience with diverse populations and disability care helpful.

Responsibilities

• Conduct comprehensive member assessments within timelines and from home visits when required.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers and providers.
• Monitor care plans and adjust as needed to achieve goals.
• Support integration of behavioral health and LTSS with community resources.
• Assess medical necessity and authorize waiver services.
• Facilitate ICT meetings for service approvals.
• Travel 25–40% to member sites as required.
