Job summary

Molina Healthcare is seeking a care management professional for remote work with travel across Dane and Jefferson Counties, WI to support LTSS members. You will collaborate with a multidisciplinary team to coordinate high-need member care and ensure progress toward outcomes.

Responsibilities include comprehensive assessments, waiver enrollment, care plan development, and ongoing monitoring. Travel up to 40% may be required depending on state contracts and member needs.

Qualifications

• 2+ years of health care experience, including LTSS and care management.
• Clinical licensure and/or certification required only if state contract mandates; active/unrestricted if licensed.
• Bachelor's degree in a health care related field may be required per state contract.
• Valid driver's license and reliable transportation for travel.
• Experience with community resources and working with diverse populations.
• Strong communication skills and ability to work independently.

Responsibilities

• Complete comprehensive member assessments within required timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement member care plans with members, caregivers, physicians and the care team.
• Monitor and adjust care plans based on outcomes and barriers.
• Promote integration of LTSS and behavioral health services across settings.
• Assess medical necessity and authorize waiver services.
• Lead or participate in ICT meetings for service decisions.
• Travel up to 25-40% for member visits as required.
