Responsibilities

• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plan effectiveness and adjust as needed.
• Promote integrated services across care teams and LTSS.
• Assess medical necessity and authorize waiver services.
• Collaborate with ICT meetings for approvals/denials.
• Educate and motivate members using motivational interviewing.
