Job summary

Qualifications

• Minimum 2 years in health care, including LTSS/care management, or equivalent experience.
• Licensing required only if mandated by state contract or regulation.
• Bachelor's degree may be required in some states.
• Valid driver’s license and reliable transportation for required travel.
• Knowledge of community resources.
• Ability to work in diverse settings and with varied populations.
• Self-motivated, proactive, detail-oriented, and able to work independently.
• Strong verbal and written communication; proficient with MS Office.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care teams.
• Monitor and evaluate care plan effectiveness, adjusting as needed.
• Promote integration of LTSS, behavioral health care, and community resources.
• Assess medical necessity and authorize waiver services as appropriate.
• Coordinate ICT meetings for service approvals/denials.
• Travel 25–40% for member visits as required.
