Job summary

Molina Healthcare is seeking a remote, field-based Care Manager RN to support LTSS members across the care continuum. You will perform comprehensive assessments, develop care plans with a multidisciplinary team, and coordinate waiver services to improve outcomes while ensuring cost-effective care.

The role requires strong communication, documentation, and the ability to travel within Black Hawk County as needed.

Qualifications

• Minimum 2 years in health care with care management or related field.
• Active RN license in state of practice.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete member assessments within regulatory timelines and home visits as needed.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and monitor care plans with members and care network.
• Monitor progress and adjust interventions to achieve goals.
• Promote integration of LTSS and behavioral health services.
