Job summary

Molina Healthcare is seeking a remote Care Manager to support LTSS coordination across Dane County, with travel to member sites as required. You will collaborate with a multidisciplinary team to assess needs and develop integrated care plans that address medical and social goals.

Responsibilities include comprehensive assessments, waiver management, and ongoing plan monitoring. The role emphasizes communication, advocacy, and coordinating resources to improve outcomes and cost-effective care.

Qualifications

• At least 2 years of health care experience, including LTSS and care management.
• Clinical licensure and/or certification required ONLY if state contract mandates, active/unrestricted if licensed.
• Bachelor's degree in a health care related field may be required depending on state contract requirements.
• Valid driver's license and reliable transportation for travel requirements.
• Knowledge of community resources.
• Ability to work with diverse populations and adjust style.
• Strong verbal and written communication skills.
• In some states, at least 1 year of experience with individuals with substance use disorders.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor care plan effectiveness and document interventions.
• Promote integration of LTSS and behavioral health services.
• Assess medical necessity and authorize waiver services.
• Evaluate benefits and funding sources.
• Facilitate ICT meetings for approval/denial of services.
• Educate and motivate members using motivational interviewing per Molina guides.
• Identify barriers and coordinate psycho/social, financial, and medical support.
• Collaborate with licensed care managers as needed.
• Travel 25-40% as required.
