Job summary

Molina Healthcare is seeking a dedicated Care Manager RN to support long-term services and supports (LTSS) across Dodge, Fond du Lac, and Green Lake Counties, WI. You will collaborate with an interdisciplinary team to coordinate care and ensure quality outcomes during member visits, including waiver-related activities.

Responsibilities include assessments, care-plan development, monitoring, and facilitating ICT meetings, with 25–40% local travel to member homes and providers as required.

Qualifications

• Registered Nurse (RN) with active license in the practicing state.
• Experience in care management, managed care, LTSS, or related health settings.
• Ability to work across settings and with diverse populations.
• Must have valid driver’s license and reliable vehicle for travel.

Responsibilities

• Perform comprehensive member assessments and home visits within required timelines.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and monitor care plans with members, caregivers, physicians, and professionals.
• Monitor care plan effectiveness, document interventions, and adjust plans as needed.
• Promote service integration across behavioral health, LTSS, and community resources.
• Evaluate medical necessity and authorize waiver services; discuss benefits funding.
• Facilitate ICT meetings for care approvals/denials; collaborate with the team.
• Conduct medication reconciliation as needed; travel 25–40% for member visits.
