Job summary

Tuesday Health is seeking a Licensed Practical Nurse (LPN) to join our Beachwood/Mentor, Ohio team. You will work with nurse practitioners and RNs to perform clinical assessments and participate in care planning that centers on the member and their caregivers.

The role requires strong communication and organizational skills, the ability to navigate multiple documentation systems, and a commitment to delivering compassionate, coordinated care in a rural and home-based setting.

Qualifications

• Active and unrestricted LPN license in Ohio.
• Experience in clinical/medical settings preferred.
• Experience in a multidisciplinary care model.
• Strong communication and organization skills.
• Ability to multitask and maintain a positive member experience.

Responsibilities

• Administer multiple assessments and screening tools within the clinical model.
• Prioritize needs based on assessment results and coordinate with the care team.
• Be accountable for care plan development within the multidisciplinary team.
• Participate in rounds to ensure a coordinated member experience.
• Learn the Tuesday Health electronic systems and tools.
• Build trusting relationships with members through empathy and active listening.
