Qualifications

• Active compact multi-state LPN/LVN license.
• At least 5 years of clinical experience as an LPN.
• Remote work readiness with stable high-speed WiFi.
• Active CA state LVN license required.
• Fluency in Spanish preferred.
• Experience educating patients with chronic conditions preferred.
• Familiarity with AI-assisted clinical tools a plus.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching.
• Coach patients to execute personalized care plans and goals.
• Conduct comprehensive assessments of health status and readiness to change.
• Educate patients on disease management, adherence, and prevention strategies.
• Monitor progress and health outcomes through remote monitoring tools.
• Document interactions and outcomes in the Cadence platform.
• Collaborate with clinicians and support teams in real time.
