Cadence is seeking a Clinical Navigator (Licensed Practical Nurse) to provide virtual chronic care support for Medicare patients. This full-time remote role involves patient outreach, education on chronic condition management, and collaboration with clinical teams to ensure high-quality patient care.

The ideal candidate will have a compact LPN/LVN license, experience in chronic care management, and proficiency with AI-assisted clinical tools.

Qualifications

• An active compact multi-state LPN/LVN license in the state where you currently reside is required.
• At least 5 years of clinical experience as a Licensed Practical Nurse is required.
• Fluency in Spanish is preferred.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one‑on‑one coaching and support.
• Educate patients on disease management, medication adherence, and treatment plans.
• Document patient interactions and identify barriers to care.
