Qualifications

• Master's degree in Social Work, Counseling, or related Behavioral Health field.
• Active, unrestricted independent Behavioral Health license.
• 3+ years post-master's direct clinical experience in behavioral health.
• 3+ years treating mental health and substance use disorders.
• 3+ years experience in crisis intervention and Motivational Interviewing.

Responsibilities

• Conduct comprehensive assessments of behavioral and physical health needs.
• Develop individualized care plans to improve health outcomes.
• Coordinate with providers, care teams and community resources.
• Apply clinical judgment to address risk factors and barriers to care.
• Utilize Motivational Interviewing to engage members and support behavior change.
• Provide crisis intervention and ensure timely access to treatment.
• Coordinate behavioral health and substance use services across the continuum of care.
• Document care and member interactions in clinical systems.
