Job summary

Qualifications

• Master's degree in Social Work or related field.
• Active, unrestricted independent behavioral health license (LCSW/LISW/LMSW/LCPC/LPC/LMFT/LMHC or equivalent).
• 3+ years post-master's direct clinical experience in behavioral health settings.
• 3+ years treating mental health and substance use disorders.
• 3+ years crisis intervention and Motivational Interviewing experience.

Responsibilities

• Conduct comprehensive assessments of members' behavioral health and physical health needs.
• Develop individualized care plans to improve health outcomes.
• Coordinate care with providers, care teams, and community resources.
• Apply clinical judgment to identify risk factors and barriers to care.
• Engage members and encourage long-term behavior change using Motivational Interviewing.
• Provide crisis intervention and ensure timely access to treatment and support services.
• Coordinate behavioral health and substance use services across the care continuum.
• Document care and member interactions in clinical systems.
