Job summary

CVS Health is seeking a Clinical Case Manager – Behavioral Health (Licensed) to support members remotely across the United States. This role focuses on assessment, care planning, crisis intervention, and care coordination for individuals with complex behavioral and physical health needs.

You will conduct comprehensive assessments, develop individualized care plans, collaborate with providers and community resources, and use Motivational Interviewing to promote sustained behavior change.

Qualifications

• Master's degree in Social Work, Counseling, or related Behavioral Health field.
• Active, unrestricted Behavioral Health license (LCSW, LISW, LMSW, LCPC, LPC, LMFT, LMHC, or equivalent).
• 3+ years of post-master's direct clinical experience in behavioral health settings.
• 3+ years of experience treating mental health and substance use disorders.
• 3+ years of crisis intervention experience and use of Motivational Interviewing techniques.
• Strong technical proficiency to document while engaging members telephonically.

Responsibilities

• Conduct comprehensive assessments of members' behavioral health, physical health, and psychosocial needs.
• Develop individualized care plans that promote improved health outcomes and well-being.
• Coordinate care with providers, care teams, and community resources.
• Apply clinical judgment to identify risk factors and barriers to care.
• Provide crisis intervention and ensure timely access to treatment and support services.
• Coordinate behavioral health and substance use services across the continuum of care.
