Job summary

CVS Health is seeking a licensed Behavioral Health clinician for a fully remote Clinical Case Manager role, supporting members with complex health needs through assessment, care planning, crisis intervention, and coordinated care. You will conduct comprehensive assessments, develop tailored care plans, collaborate with providers and community resources, and document encounters using telephonic workflows.

A Master’s in Social Work and an active license are required.

Qualifications

• Master's Degree in Social Work, Counseling, or a related Behavioral Health field.
• Active, unrestricted independent Behavioral Health license (LCSW, LISW, LMSW, LCPC, LPC, LMFT, LMHC, or equivalent).
• 3+ years post-master's direct clinical experience in behavioral health settings.
• 3+ years of experience treating mental health and substance use disorders.
• 3+ years of experience in crisis intervention and Motivational Interviewing.

Responsibilities

• Conduct comprehensive assessments of members' behavioral health, physical health, and psychosocial needs.
• Develop individualized care plans that promote improved health outcomes.
• Collaborate with providers, care teams, and community resources to coordinate care.
• Apply clinical judgment to identify and address risk factors and barriers to care.
• Utilize Motivational Interviewing to engage members and promote behavior change.
• Provide crisis intervention and ensure timely access to treatment.
• Coordinate behavioral health and substance use services across the continuum of care.
• Document care and manage member interactions in clinical systems.
