Job summary

Vynca Inc. is seeking a Lead Care Manager (LCM) to coordinate care for individuals with complex needs. The LCM partners with healthcare providers, social services, and family caregivers to ensure timely access to services and cost-effective care delivery.

The role emphasizes person-centered care and comprehensive care planning across multiple health domains. Reporting to ECM leadership, the LCM oversees care plans, facilitates access to resources, and maintains EMR/CRM documentation while

Qualifications

2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for evenings and weekends.
Bilingual English/Spanish preferred.
Knowledge of government and community resources related to social determinants of health.
General computer skills and familiarity with the internet.

Responsibilities

Assess member needs across physical health, mental health, SUD, oral health, palliative, memory, trauma-informed care, social supports, housing, and community services.
Oversee development of client care plans and goal setting.
Connect clients to social services and advocate with healthcare professionals.
Document outcomes, complete CRM and EMR entries within plan timeframes.
Attend training as assigned.
