Job summary

Vynca Inc. is seeking a Lead Care Manager to coordinate complex care for clients, partnering with clinicians, social services, and family caregivers. The role emphasizes person-centered planning, timely access to services, and collaboration with a dedicated care team in a remote setting.

The LCM will manage care plans, drive SMART goals, and ensure documentation, reporting, and program compliance. A bilingual candidate with strong communication and transportation assets is preferred.

Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker.
• Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for potential evenings and weekends.
• Working knowledge of government and community resources related to social determinants of health.
• Clean driving record, valid driver's license, and reliable transportation.
• Excellent oral and written communication skills.
• Bilingual (English/Spanish).

Responsibilities

• Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referrals to community services.
• Oversees the development of client care plans and goal setting.
• Offer services where the member resides, seeks care, or is most accessible (office, telehealth, field-based).
• Connect clients to social services and supports as needed.
• Advocate on behalf of the client with health care professionals.
• Utilize evidence-based practices including Motivational Interviewing and Trauma-Informed Care.
• Conduct outreach and log activity in the CRM.
• Evaluate progress and update SMART goals.
• Provide mental health promotion.
• Arrange transportation (e.g., ACCESS).
• Complete documentation and outcome measures on time.
• Maintain EMR/health records.
• Complete monthly program reporting.
• Attend training as assigned.
