Job summary

Molina Healthcare seeks an IRIS Consultant to build relationships with participants enrolled in the Wisconsin IRIS program. This remote role involves home-based coordination and regular in-person visits to support goal attainment in areas like employment, housing, health, and safety.

You will identify participant outcomes, create personalized IRIS plans, and navigate budget considerations while ensuring compliance and confidentiality.

Qualifications

• Minimum 2 years health care experience, care coordination preferred.
• Bachelor’s degree or four years of commensurate experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Build relationships with IRIS participants and navigate the Wisconsin IRIS program.
• Identify long-term care goals and develop customized IRIS plans.
• Coordinate visits; meet in-person with participants at least four times per year.
• Educate participants on budget reports and program rules; liaise with Fiscal Employer Agency.
• Maintain confidentiality and HIPAA compliance; document activities in WISITs.
• Collaborate with agency staff to implement plans and attend team meetings.
