Job summary

Molina Healthcare is seeking a compassionate IRIS Consultant to assist individuals in navigating the Wisconsin IRIS Program. This remote role requires building relationships with participants and supporting them in achieving their long-term care goals.

The successful candidate will have at least 2 years of healthcare experience and a Bachelor’s degree in a related field. Responsibilities include providing program orientation, developing customized plans, and maintaining compliance with confidentiality standards.

Qualifications

• Minimum 2 years experience in health care, preferably in care coordination.
• Bachelor’s degree or equivalent experience required.
• Valid driver's license and reliable transportation required.

Responsibilities

• Build relationships with IRIS participants and help them navigate the program.
• Document orientation and planning activities in the IRIS data system.
• Attend in-person monthly team meetings and participate in ongoing training.
