Job summary

Lutheran Social Services of Wisconsin & Upper Michigan in Eau Claire/Chippewa, WI (Hybrid) seeks an IRIS Consultant to support adults, older adults and individuals with disabilities through Wisconsin's IRIS program. This full-time, 40 hours/week role travels 30–50% and emphasizes self-directed, person-centered care.

You'll build relationships, coordinate services, document thoroughly, and collaborate with ADRCs, healthcare providers, and community partners to promote independence and safety.

Qualifications

• Bachelor's degree in Social Work or related field with at least one year of supervised experience with older adults or individuals with disabilities, OR four or more years of direct experience, OR current IRIS Consultant in good standing with Wisconsin DHS exemption.

Responsibilities

• Develop and maintain trusted relationships with IRIS participants
• Assist participants in creating and updating individualized IRIS plans
• Conduct monthly participant contacts and quarterly in-person visits
• Promote participant choice, self-direction, independence, and community inclusion
• Monitor participant health, safety, and program outcomes
• Educate participants about IRIS enrollment requirements, eligibility, and resources
• Collaborate with ADRCs, healthcare providers, and service providers
• Maintain accurate case documentation in required systems
• Complete documentation within compliance standards and timelines
• Assist with annual Long-Term Care Functional Screens when applicable
• Work independently while collaborating with an interdisciplinary team
