Job summary

TMG is seeking an IRIS Consultant to help Wisconsin IRIS participants navigate the program and achieve personal goals. The role is remote with regular in‑person visits to participants’ homes and communities, balancing office work with field responsibilities.

You will identify outcomes, develop plans for employment, housing, health and safety, and document progress in the specified data system while maintaining confidentiality and HIPAA compliance.

Qualifications

• At least 2 years experience in health care, preferably in care coordination.
• Bachelor’s degree in a social work, psychology, human services, counseling, nursing or related field.
• Valid driver’s license, reliable transportation, and adequate auto insurance.
• Ability to work independently with minimal supervision and self-motivation.
• Knowledge of long-term care programs and self-determination principles.
• Strong problem-solving, time management and prioritization skills.
• Excellent verbal and written communication skills.
• Familiarity with HIPAA and confidentiality requirements.
• Proficiency with Microsoft Office and related software.

Responsibilities

• Meet IRIS participants in person per year, including at least four in‑person visits.
• Provide program orientation and explain rights and responsibilities of IRIS.
• Identify personal outcomes and develop customized IRIS plans.
• Document all activities in the IRIS data system (WISITs) within 48 business hours.
• Explain monthly budget reports and coordinate with Fiscal Employer Agency.
