Job summary

Molina Healthcare is seeking an IRIS Consultant to support Wisconsin participants in the IRIS program. This remote role involves home visits and community-based support, helping participants navigate long-term care options and achieve personal goals.

You will build trusted relationships, manage budgets within the IRIS framework, and collaborate with staff across the IRIS Consultant Agency to deliver high-quality services while maintaining confidentiality and HIPAA compliance.

Qualifications

• Minimum 2 years of health care experience with care coordination background.
• At least 1 year serving IRIS program or similar community-based populations.
• Valid driver's license and reliable transportation for field visits.
• Ability to work independently with minimal supervision.

Responsibilities

• Meet IRIS participants in person at least four times per year and annually in their home.
• Develop customized IRIS plans to achieve goals in employment, housing, health, and safety.
• Document all activities in the IRIS data system within 48 business hours of visits.
• Coordinate with family, providers, and regulators to ensure program compliance.
