Qualifications

• 2+ years in health care, care coordination preferred.
• Bachelor's degree in social work or related field or 4 years relevant experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Meet IRIS participants in homes and communities regularly.
• Document activities in IRIS data system within 48 hours.
• Educate participants on budgeting and program options.
• Develop and maintain relationships with participants and vendors.
• Attend monthly team meetings and participate in weekly check-ins.
