Job summary

Molina Healthcare is seeking an IRIS Consultant to support participants in the Wisconsin IRIS program. This remote role involves home visits with participants and close collaboration with the IRIS Consultant Agency to create customized plans.

The position emphasizes compassion, reliability, and strong relationship-building with diverse populations. Responsibilities include coordinating services, educating participants on budget and program options, and documenting progress in the IRIS data

Qualifications

• 2+ years experience in health care, preferably care coordination
• 1+ year experience serving IRIS target groups or equivalent
• Bachelor’s degree in a related field or four years of commensurate experience
• Valid driver's license and reliable transportation for travel requirements
• Ability to develop positive relationships with coworkers, clients, and providers
• Independent work capability with minimal supervision
• Knowledge of long-term care programs and self-determination principles
• Strong time-management and prioritization skills
• Ability to handle multiple projects and adapt to change
• Excellent written and verbal communication skills
• Proficiency with office software programs

Responsibilities

• Meet IRIS participants in person at least four times per year (one annual home visit)
• Provide program orientation and explain participant rights and responsibilities
• Identify participant goals and create plans to achieve IRIS outcomes
• Document activities in the IRIS data system (WISITs) within 48 business hours
• Research community resources and supports to fit participant outcomes
• Educate participants on budget reports and connect with providers, brokers, and vendors
• Maintain confidentiality and HIPAA compliance
• Collaborate with IRIS staff to ensure successful plan implementation
• Attend monthly in-person team meetings and participate in periodic check-ins
