Job summary

Molina Healthcare is seeking an IRIS Consultant (IC) to support Wisconsin IRIS participants remotely from home, with regular home visits. You will partner with individuals to identify goals related to employment, housing, health, safety, community involvement, and transportation.

As an IC, you will build trust, document activities in the IRIS data system, and coordinate resources across community supports while ensuring HIPAA compliance and confidentiality.

Qualifications

• At least 2 years experience in health care, preferably in care coordination.
• Bachelor's degree in social work or closely related field.
• Valid and unrestricted driver's license with reliable transportation for travel.

Responsibilities

• Build relationships with IRIS participants and help them navigate the Wisconsin IRIS program.
• Meet with participants in-person at least four times per year and one annual home visit.
• Document orientation and planning activities in the IRIS data system within 48 business hours of visits.
• Develop and adjust customized IRIS plans to help participants achieve their goals (employment, housing, health, safety, etc.).
• Educate participants on budgeting and interpret monthly budget reports; coordinate with the Fiscal Employer Agency and IRIS Consultant Agency.
