Job summary

Molina Healthcare is seeking an IRIS Consultant to support Wisconsin IRIS participants remotely. You will visit participants in their homes and communities, helping them navigate the long-term care program and meet personal goals.

Strong communication, relationship-building, and budget management are essential as you connect people to community resources and coordinate with the IRIS team. Prior care coordination experience is preferred; a valid driver's license is required.

Qualifications

• 2+ years in health care, care coordination preferred, plus 1+ year with IRIS target groups.
• Bachelor's degree in social work or related field, or four years' commensurate experience.
• Valid driver's license and reliable transportation for travel.
• Ability to work independently and build relationships with diverse stakeholders.
• Experience with community resources and self-determination principles.

Responsibilities

• Build trusting relationships with IRIS participants and help navigate the program.
• Coordinate in-home and community visits as part of care planning.
• Help participants develop customized IRIS plans for goals (employment, housing, health, safety, transportation).
• Document orientation and planning activities in the IRIS data system within 48 business hours of visits.
• Educate participants on budget reports and assist with provider billing or related processes.
• Collaborate with IRIS Consultant Agency staff and attend monthly in-person meetings and weekly check-ins.
