Job summary

Molina Healthcare in Wisconsin seeks an IRIS Consultant (IC) to build trusted relationships with participants in the Wisconsin IRIS Program. This remote position is home-based, with regular in-person visits in participants’ homes and communities.

You will help clients identify long-term care goals, navigate the IRIS program, coordinate resources, and develop customized IRIS plans while staying within budget and ensuring confidentiality and HIPAA compliance.

Qualifications

• At least 2 years experience in health care, preferably in care coordination.
• At least 1 year experience serving target IRIS groups (adults with disabilities or older adults).
• Bachelor’s degree in a social work, psychology, human services, counseling, nursing, special education, or closely related field (or four years of commensurate experience).
• Valid and unrestricted driver’s license with reliable transportation for job-related travel.

Responsibilities

• Meet IRIS participants in person at least four times per year, including one annual home visit.
• Provide program orientation about rights and responsibilities in IRIS.
• Develop customized IRIS plans and connect participants to community resources.
• Document orientation and planning activities in IRIS data system (WISITs) within 48 business hours.
• Educate participants on budgeting and help manage IRIS budget within program rules.
• Maintain confidentiality and HIPAA compliance.
• Collaborate with IRIS Consultant Agency staff to implement participant plans.
