Job summary

The Intake Care Representative at CarepathRx, part of Evernorth Health Services, serves as the first point of contact for patients, providers, hospitals, and referral partners. You will guide callers through the referral and intake process, gather required information, and ensure timely care initiation.

Hours are Mon-Fri 9am-6pm EST with rotating shifts; some weekend/holiday coverage. The role emphasizes compassionate, patient-centered service within a healthcare team and may involve remote work

Qualifications

• High school diploma or GED required.
• 1 year of healthcare/related experience preferred.
• Knowledge of medical terminology.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office (Outlook, Word, Excel).
• Ability to manage multiple priorities in a fast-paced environment with attention to detail.

Responsibilities

• Serve as the primary point of contact for patients, responding to calls and providing timely, professional support throughout the intake process.
• Help patients understand next steps, gather required information, and connect them with appropriate care teams.
• Handle inbound and outbound communication with patients, providers, hospitals, and offices regarding referrals and care coordination.
• Support timely patient onboarding by gathering, reviewing, and entering referral information into systems.
