Job summary

The Intake Care Representative role sits within CarepathRx, part of Evernorth Health Services, and serves as the first point of contact for patients and providers during intake. You will gather information, explain steps, and coordinate with care teams to streamline referrals and onboarding.

This position offers Monday–Friday hours with some weekend/holiday coverage, potential home-based work, and eligibility for an annual bonus.

Qualifications

• High school diploma or GED required.
• 1 year of healthcare-related experience preferred.
• Knowledge of medical terminology beneficial.
• Strong verbal and written communication skills required.
• Able to multitask in a fast-paced environment while maintaining accuracy.
• Proficiency with Microsoft Office (Outlook, Word, Excel).
• Commitment to patient confidentiality and privacy.

Responsibilities

• Serve as primary contact for patients, providing timely support during the intake process.
• Explain next steps, collect required information, and connect patients with care teams.
• Handle communications with patients, providers, hospitals, and offices regarding referrals and care coordination.
• Assist with onboarding by gathering and entering referral information accurately.
• Review and route referral packets and medical orders to appropriate teams.
• Coordinate home health services based on patient needs and insurance requirements.
• Verify treatment timelines and scheduling for first-dose and home infusion.
• Maintain accurate patient records in the medical record system.
• Collaborate with Intake Coordinators and nurses to move referrals efficiently.
• Communicate updates to prevent delays in care.
• Support administrative tasks, phone coverage, and document distribution.
