Job summary

Mosaic Life Care is seeking an Inpatient Coder II to assign ICD-10-CM and ICD-10-PCS codes for acute care inpatient services, including rehabilitation and LTACH. This remote role supports facilities across multiple states and emphasizes accurate documentation and coding guidelines.

The successful candidate will review medical records, resolve coding denials when needed, and collaborate with care teams to ensure compliant, timely coding and reported data quality.

Qualifications

• Must have coding education (required).
• Associate's Degree in Health Information Management / Medical Records (required).
• Bachelor's Degree in Health Information Management / Medical Records (required).

Responsibilities

• Codes complex diseases, procedures, and diagnoses using ICD-10-CM/PCS per guidelines.
• Mentors and participates in professional development activities.
• Reviews and appeals coding denials; completes complex coding assignments for reimbursement and compliance.
• Educates/providers to ensure complete and accurate documentation for coding.
• Coordinates with clinical departments and care teams to ensure timely and accurate coding.
