Job summary

Mosaic Life Care seeks an Inpatient Coder II to assign ICD-10-CM and ICD-10-PCS codes for acute care inpatient services, including LTACH. You will query documentation, resolve edits, and support education for coders within a multidisciplinary team.

Requirements include HIM coding education, familiarity with disease processes, and coding guidelines. This role emphasizes accuracy, compliance, and ongoing professional development.

Qualifications

• Must have coding education with knowledge of medical terminology, pharmacology and anatomy, query guidelines - Required
• Associate's Degree - Health Information Management / Medical Records - Required OR
• Bachelor's Degree - Health Information Management / Medical Records - Preferred

Responsibilities

• Codes ICD-10-CM/PCS for inpatient services including LTACH and rehabilitation.
• Queries providers to improve documentation and demonstrate severity of illness.
• Mentors and trains coders; collaborates with clinical documentation integrity teams.
• Maintains regulatory-compliant coding across audits and edits.
