Job summary

Mosaic Life Care is seeking an Inpatient Coder II to assign ICD-10-CM/PCS codes for acute inpatient services, including LTACH, and to query physicians for documentation clarity. The role emphasizes collaboration with clinical documentation integrity and revenue cycle teams to ensure accurate coding and reporting.

Requires strong coding education, HIM/medical records background, and 3+ years of inpatient HIM coding experience.

Qualifications

• Must have coding education with knowledge of medical terminology, pharmacology and anatomy, query guidelines - Required
• Associate's Degree - Health Information Management / Medical Records - Required
• Bachelor's Degree - Health Information Management / Medical Records - Preferred

Responsibilities

• Codes complex diseases, procedures and diagnoses using ICD-10-CM/PCS.
• Queries providers following Official Query Guidelines for documentation quality.
• Assigns MS-DRG and APR-DRG in line with guidelines.
• Mentors and trains coders.
• Resolves intermediate coding edits and ensures data accuracy for regulatory reporting.
