Pacific Health Group in Santa Cruz County is seeking a field-based Housing Stability Specialist to support Medi-Cal members through CalAIM Community Supports housing programs. You will navigate housing resources, coordinate services, and guide members toward long-term housing stability.

This role emphasizes independent fieldwork, strong organization, and collaboration with housing providers, health teams, and community organizations to improve access and outcomes.

Qualificações

• Strong understanding of the housing system and local housing resources.
• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs preferred.
• Excellent communication, empathy, and organizational skills.
• Proficient with technology, EHR systems, and real-time documentation.

Responsabilidades

• Support Housing Transition Navigation Services and Housing Tenancy & Sustaining Services.
• Coordinate housing deposits and landlord engagements to secure long-term housing.
• Conduct field-based visits, assessments, and follow-ups in community settings.
• Build partnerships with housing providers, shelters, and community organizations.
• Document interactions, track outcomes, and ensure regulatory compliance.
