Job summary

Chapters Health System is seeking a Resource Nights & Weekend Nurse Practitioner (ReNW NP) to provide fully remote serious illness care management to patients and families across our continuum, including home-based, facility-based, and inpatient hospice, palliative care, PACE, and related programs.

The ideal candidate has at least 3 years of relevant clinical experience, a Master’s or Doctoral degree in Nursing (Family or Adult NP), and national certification with RN compact licensure.

Qualifications

• Must have minimum 3 years of clinical experience in hospice, palliative care, or acute inpatient care.
• Master’s or Doctoral degree in Nursing from an accredited Family or Adult NP program.
• Must hold national certification and compact RN license in home state and other jurisdictions.

Responsibilities

• Provide fully remote virtual serious illness care management to patients and families.
• Consult with medical staff to ensure optimal patient care.
• Perform virtual assessments and manage medications/equipment as appropriate.
• Document interactions in EMR and maintain prescribing orders.
• Support discharge planning and participate in QA/QI activities.
