Job summary

Chapters Health System, Inc. is seeking a Resource Nights & Weekend (ReNW) Nurse Practitioner to provide fully remote serious illness care management across our continuum of care, including hospice, palliative care, and related programs. This role requires licensure in home state and other required jurisdictions, with BLS and NP certification.

Qualifications

• Minimum 3 years of clinical experience in hospice, palliative care, or acute inpatient care.
• Master's or Doctorate in Nursing from an accredited Family or Adult NP program.
• National certification, state licensure, and prescriptive authority for advanced nursing practice in home state and others; BLS certification; compact RN license.

Responsibilities

• Provide fully remote virtual serious illness care management to patients and families.
• Assess patient/family needs and prescribe medications/equipment as appropriate.
• Coordinate and document care in EMR; participate in discharge planning and QA/QI activities.
• Collaborate with the interdisciplinary team to ensure continuous quality care.
• Maintain licensure and knowledge aligned with regulatory requirements.
