Job summary

Chapters Health System, Inc. is seeking a Resource Nights & Weekend (ReNW) Nurse Practitioner to provide fully remote virtual care management for seriously ill patients and families across our care continuum, including hospice and palliative services.

The role requires an NP with 3+ years of clinical experience in hospice, palliative, or acute inpatient settings, and a Masters/Doctorate in Nursing with national certification and prescriptive authority.

Qualifications

• Masters or Doctorate in Nursing from an accredited Family or Adult NP program.
• Must hold national certification and prescriptive authority in home state and other jurisdictions.
• Basic Life Support (BLS) and compact RN license required by date of hire.

Responsibilities

• Provide fully remote virtual serious illness care management to patients and families.
• Consult with medical staff and IDT to ensure optimal patient care.
• Perform virtual assessment of patient/family needs and adjust care plans accordingly.
• Prescribe medications and equipment as appropriate within scope of practice.
• Maintain EMR access and document interactions promptly in the EMR.
• Support discharge planning by entering orders and instructions.
