Job summary

The Resource Nights & Weekend (ReNW) Nurse Practitioner (NP) at Chapters Health System is a remote role delivering serious illness care management to patients and families across home-based, facility-based, and inpatient hospice and palliative care settings.

Applicants must have at least 3 years of clinical experience in hospice/palliative or acute inpatient care, and hold a Master’s or Doctoral degree in Nursing from an accredited Family or Adult NP program, plus national certification and

Qualifications

Minimum 3 years of clinical experience in hospice, palliative care, or acute inpatient care.
Master’s or Doctoral degree in Nursing from an accredited Family or Adult NP program.
National certification, licensure, and prescriptive authority in home state and other required states.
Basic Life Support (BLS) and compact RN license in home state by date of hire.

Responsibilities

Provides fully remote virtual serious illness care management to patients and families across our continuum of care.
Consults with medical staff to ensure optimal patient care.
Performs virtual assessment of patient/family needs.
Identifies, manages, orders, and prescribes medications and equipment as appropriate.
Maintains EMR access and timely documentation.
Supports discharge planning with orders and instructions.
Participates in QAPI activities and pursues quality care improvements.
